Traffic Control Plan
Provide a sketch or diagram indicating the location of Road Occupancy and Traffic Control Plan. Show appropriate signage for lane closures and detours for review and comment only. Not for approval.

Per OTM Book 7 Section 1.3:
Workers who design traffic control plans to protect both workers and road users: a) Shall be a competent worker; b) Shall be knowledgeable in standards and guidelines of OTM Book 7 and the Regulations for Construction Projects; c) Shall be able to recognize the design elements of work zone traffic control; d) Shall be given adequate training with respect to techniques and procedures for designing effective, efficient and safe traffic control plans.
A Competent worker means a worker who: • Is qualified because of knowledge, training and experience to perform the work; • Is familiar with the Occupational Health and Safety Act and with the provisions of the regulations that apply to the work; and • Has knowledge of all potential or actual danger to health or safety in the work.
Insurance Document
Provide a copy of General Liability Insurance below as per Guidelines naming The Township of Wilmot as additional insured.
Declaration of Applicant
I/We hereby make application to occupy the designated space for the purpose described above and agree to abide by the terms and conditions established in this application. I/We agree to assume all liability and/or costs incurred by the Township as a result of occupying the designated space and to indemnify and save harmless the Township until final approval of the works. I/We understand that the issue of a e responsibility of complying with all relevant legislation and municipal by-laws.
